The difference between in-network and out-of-network — in real dollars

March 4, 2026·4 min read

On paper, "out-of-network" sounds like a small inconvenience. In reality, a single ER visit can swing $4,000–$12,000 depending on whether the hospital, the ER doctor, and the radiologist are all in your plan's network.

What we see on real EOBs

  • In-network ER: copay or coinsurance applies, balance billing is not allowed.
  • Out-of-network ER (emergency): No Surprises Act protects you in many cases — but not all.
  • Out-of-network specialist follow-up: full sticker price unless you check first.

The one habit that saves people the most money: before any scheduled procedure, call the plan and confirm every provider involved — surgeon, anesthesiologist, facility — is in-network.

Coverage that fits — without the runaround.
